Telehealth Informed Consent

Effective date: August 26, 2026 · HOO Longevity is operated by Hoo AI Agency LLC · Contact: support@hoolongevity.com · (586) 321-3377

Purpose

This consent explains telehealth so you can decide whether to receive care this way. Telehealth means your provider evaluates and treats you remotely: through the written intake, asynchronous messaging, and, where required or appropriate, phone or video.

What to expect

You will complete a health questionnaire; a licensed provider will review it, may message you with follow-up questions, and will determine whether treatment is appropriate. If prescribed, medication ships from a licensed U.S. pharmacy. Ongoing check-ins are part of your program.

Benefits and risks

Benefits include access, convenience, and speed. Risks include the limitations of care without an in-person physical exam, the possibility of delays or technology failures, and rare gaps in transmitted information. Your provider may determine telehealth is not appropriate for you and refer you to in-person care. Telehealth is not for emergencies. If you experience a medical emergency, call 911.

Your rights

Your participation is voluntary. You may withdraw consent, stop treatment, or request your records at any time without affecting your right to future care. You are entitled to know the identity and licensure of the provider who treats you. Medical services are provided by an independent, U.S.-licensed medical practice.

Privacy

Your health information is handled under our Privacy Policy and HIPAA Privacy Notice. Reasonable safeguards, including encryption in transit, protect your information.

Consent

By completing an online visit, you acknowledge that you have read and understood this consent, that you can ask questions before treatment, and that you consent to receive care via telehealth.

Rx programs require an online visit with a licensed provider · results vary.
